Case Management

Case Management Services are used to enhance the independence of the client who may be struggling with severe or persistent mental illness. Case Managers help with coordinating service, advocating for clients, and supporting clients in achieving goals.

 

Case management service is the “glue” that binds together a coordinated plan of treatment and support services for persons with severe mental illness.

A Case manager will assist in:

* Assessments

* Planning

* Linkage

* Advocacy

* Coordination

* Monitoring to assist in gaining access to needed health and dental service, financial assistance, housing ,employment ,education, and other natural supports developed through the person centered planning process.
